Remaining Balance Distribution	The minimum portion of President’s Club dues allocated to the OAJ operating account is $2500, the minimum portion of Eagles Club dues allocated to the OAJ operating account is $5000. If you elected either of these membership categories, please tell us how you would like the remaining portion of your dues distributed.

Note: Applications are reviewed by the OAJ Executive Committee on a monthly basis. Upon acceptance of your application for membership, an information packet will be mailed to you outlying benefits and services. Credit card charges or automatic debits will not begin until approval for membership by the Executive Committee. In the event your application is denied, any cash or check payments will be reimbursed to you within three business days of application rejection.

By submitting this form, I am committed to the preservation of the adversary system, to the right to trial by jury and my practice is consistent with the purposes and objectives of the OAJ.
